cannot format Transcend SDXC card

This particular card will not format in my Nikon D5200 nor will it format in my Mac. Disk Utility tells me it succeeded, but the old name of the card returns along with 2 NEF images in a DCIM folder. It 128GB 600X card. Just purchased a couple of weeks ago and, when trying to format in camera, it says that the card may be damaged and try another card. I tried an old 16MB card which works fine. I also tried various format types on the Mac- ExFAT, Mac, and MS-DOS FAT. No success.
